Visible Men Qualification Form

Thank you for your interest in becoming a Visible Man!  As a potential role model for black boys, your involvement in this movement is critical.  To be considered as a Visible Man, you must agree to the Statement of Character below and to a background check (release at the end of this form).  In order to perform a thorough background check, we ask for traditionally-private information, like your Social Security Number, former employers, etc.  To protect your privacy, this form is encrypted and Visible Men will not disclose this information to any third party beyond what is necessary to conduct the background check.  Your private information will never be sold.

Statement of Character

As a Visible Man, I recognize that I am declaring myself as a role model for others, particularly young people, to emulate. I am aware of the significance of this declaration and proudly accept the responsibility of past, present, and future personal, professional, and social conduct that accompanies it.
